LTO Double License

Anybody here na meron double license na problem sa LTO?  Yun nadetect ng system na dalawa ang license mo at magkaiba ang license number, Anybody who also have this problem?  If yes, paano po kaya ninyo inayos ito?  I have a cousin who have this problem.  Thank you.
